Picture Identification
Insurance Cards
All medication you are currently taking (if any)

If you do not have medical insurance, El Dorado Community Health Center offers a sliding fee scale program to patients within a certain income level. to help us determine your eligibility for the sliding fee program, in addition to the previous forms, you will need to bring one of the following documents to show proof of income;

Pay stub
bank statements
tax return
